What to bring for your new appointment

  • Insurance card and your indentification (e.g. driver's license)

  • Referral lett​er from your primary physician (if required by insurance)

  • New patient forms

  • Current medications (including over the counter medicines, vitamins and herbal supplements)

  • Medical records relevant to your visit (charts, past skin test results, CT scan reports, etc).
